Reducing the risks of diabetes

We work with people at risk of developing Type 2 diabetes, through our delivery of Healthier You: the NHS Diabetes Prevention Programme. Our participants have usually been referred to us by their GP or a health professional.

The community-based programme is informal, interactive and supportive. Over nine months of support, people start to feel healthier and make sustainable changes to improve their health and lifestyle – and reduce their risk of disease.

We focus on support to achieve or maintain a healthy body weight, adopt a healthy balanced diet, increase physical activity and reduce sedentary behaviour.

Managing Type 2 diabetes

We help people with Type 2 diabetes to achieve and maintain a healthy body weight, adopt a balanced diet, increase physical activity and reduce sedentary behaviour.

By doing this, participants can better control their blood sugar levels, reduce medication, prevent disease and achieve remission.

Our support is delivered through the NHS Type 2 Diabetes Path to Remission programme. Over 12 months of support, those using the service are supported by trained Health Coaches to make sustainable lifestyle changes.

The programme includes a period of total food replacement, followed by reintroduction and lifestyle maintenance.

The aim is to put Type 2 diabetes into remission so that people’s blood sugar levels remain healthy without having to take any medication.

13% more weight loss on average

By tailoring and targeting our programmes, we’ve helped people achieve better results than with a ‘one-size-fits-all’ approach.

In fact, the official NHS Diabetes Prevention Programme performance review found that our average participant weight loss was 13% greater than the next best face-to-face provider across all participant groups, including those from deprived communities and ethnic minority backgrounds.
